This is truely a problem for me – anytime I touch an image by Gimp or other means the problem of outdated thumbnails occurs. Anytime images are deleted or moved in the filemanger, shotwell complains their miss – automagic or manually invoked database updates is imho an important feature – other tickets concerning this are http://trac.yorba.org/ticket/1941 and http://trac.yorba.org/ticket/374

I agree: this is a significant limitation with Shotwell today. When we implement #374 (closed), Shotwell will automatically update thumbnails for any image which has changed externally. Similarly, if and when we implement #1941 , Shotwell will update thumbnails upon request for any image which has changed externally at the same time that it scans for new or deleted images.

I believe this ticket is now fixed due to startup scan and monitoring (now in trunk, to be available in 0.8).
If the user wants to be able to manually refresh the thumbnail (via a command), that's a different matter. Probably should open a new ticket for that.
