Presumably this is really a side effect of #4034 (closed) (moving a tag in hierarchy is not undoable), and the right way to fix this is to fix that bug.

The first time ctrl-Z is pressed, you get a new top-level tag called "Untitled." The second time you get the assertion. This appears related to tag reconstitution.

The issue seems to be that when a tag is re-parented, the original tag is deleted. Undoing after the re-parent undoes the rename (i.e. from "cow" back to "untitled.") The restored tag exists side by side with the un-reparented tag.
