Add issue template for string freezes

This is an attempt at moving the process to gitlab. Let me know what you think.

There will of course be many places and references to fix once we merge this (Update documentation on string freeze exceptions (#3 - closed)), but the first step is getting this here done.

Review/feedback from @dmustieles @piotrdrag @claudep would be appreciated.
