Draft: Restructure the documents

I was really struggling with the existing documents - I kept looking at them and was still unable to work out what all the procedures exactly where. The changes in this MR makes it clearer to me at least.

This MR is a proof of concept, and it would be great to get feedback. If the general direction is OK, I'll work up a proper set of commits so the changes can be seen more easily.
