I'm seeing this behavior in Shotwell 0.7 on both Lucid and Maverick.
To see the problem:
Open a photo in full-screen view.
Wait for the toolbar to disappear.
Move the mouse all the way to the bottom of the window, so that the toolbar reappears.
Move the mouse up slightly so that it is over of the the toolbar buttons.
The toolbar will likely vanish, even though the mouse is over it. If you don't see this bug at first, perform steps 3 and 4 repeatedly and you will see the bug before long.

I've found I can easily reproduce this bug when Metacity is my window manager, but it does not seem to occur when the window manager is Compiz.

So the issue with the toolbar turned out to be that Metacity returns (on my system) a 29-pixel y offset from the top of the screen to window. This offset is why holding your cursor over the top part of the toolbar caused it to go away after a while, but holding the cursor near the very bottom of the toolbar makes it stay on the screen.
