Japanese NFD text rendered incorrectly (implement grapheme-based font fallback)

Submitted by Mikachu Link to original bug (#753400)
Description
For example with firefox on https://www.youtube.com/watch?v=DPS3loXzGTw the ゛ on the ケ is rendered on top of the ハ instead. Copying the text to a terminal and back into firefox renders it correctly, because my terminal converts it to NFC. The bug is not specific to firefox; Openbox also uses pango and renders the title of the window of that same url in the same way, with the ゛ on the wrong character. (The characters in the artist part in the same title is already NFC on youtube and are therefore rendered correctly).
