Use fallbacks for missing window titles

Florian Müllner requested to merge wip/fmuellner/use-fallback-titles into master

Not all windows set titles, but as we don't specifically handle that case, we end up with blank window captions and empty menu items in app icons' context menus.

Fall back to the app name in that case.
