Multi monitor fail whale dialog

If gnome-shell fails running because of a failure of a required application, and we're in a multi-monitor setup the shell might be still usable and the user won't be prompted anywhere about the issue.

So just try to open a dialog in each monitor.

Also modernize and cleanup the code in previous commits.
