Bugs in handling PDF form

Description

The French government's passport request form has a couple of bugs in evince: http://vosdroits.service-public.fr/particuliers/R1220.xhtml (also attached)

  1. The top buttons don't do anything
  2. The justification for the 2nd surname cannot be entered ("Précisez s'il s'agit du nom de votre:")
  3. Despite being entered, the form doesn't show the content in the field for father's surname ("Père" -> "Nom") ou the birthdate ("Né le" -> "J")
  4. The date at the bottom of the first page is pre-filled with 19-07-2011 and is not editable
