Remove annotations toolbar and a zillion cleanups in the annotation-related code

This started as just removing the annotations toobar, but got a bit out of hand, sorry if it's annoying to review.

This is how the new window looks:

Unfortunately, in that commit there seems to be something strange, since I'm getting windows opened maximized. Would be nice to get some feedback of whether that's the case for you too.
