Update "Session Modes" topic page

The "Seesion Modes" page includes an example that hasn't been reviewed in some time and may contain misleading advice. Revisit the page and update the example code.

Also update the relevant documentation in the "Anatomy" page to detail how extensions can be re-based depending on the situation.
