mobile-data-quick-setting: Mark as insensitive when modem is disabled

It looks a bit odd if we have it enabled while the modem is off.

Clean up around the edges a bit while at that.

I opted to not disable the quick setting on the lock screen for the moment as it doesn't allow other devices to connect.
