Windows are often placed outiside of the monitors' visible area on some multi-monitor setups

Bug summary
Mutter often places windows outside of the visible monitor area. It usually happens when I plug in or unplug a monitor, and existing videos get moved outside of the monitors - but sometimes it happens even just by opening an application.
Steps to reproduce
- Set up two monitors
- Open a video in MPlayer
What happened
Window gets placed outside of the visible area
What did you expect to happen
Mutter should restrict windows to the monitor area (even if it means overriding the app's requested location)
Relevant logs, screenshots, screencasts etc.
Here is a screencast. The black rectangles in the top left and bottom right are not visible on the monitors.
