Bangladeshi provider information update

  1. Removed bankrupt provider CityCell

  1. Updated profile names to "{Provider} Internet/Web/MMS" so users with multiple profile can select correct one based on name

  2. Fixed "Teletalk MMS" from incorrect "Airtel MMS"

  3. Removed GP Internet username, password, DNS. Based on "APN" provider will auto-configure the connection. No need to hardcode it.
