  initGenericErrorDefaultFunc() was really wrong.
* xmlreader.c include/libxml/xmlreader.h: xmlTextReaderMode enum
  must be made public, added some missing comments on the XMLReader
  header.
* c14n.c: Alexsey fixed C14N bug with processing namespaces
  from attributes
