Add annotations to all exported functions in GTK+ headers.

gcc has optimizations for include guards that only work
if they are outermost in the the header.

Use a private pointer instead

This commit adds constructors for GtkPaned, GtkBox, GtkButtonBox,
GtkRuler, GtkScale, GtkScrollbar and GtkSeparator and makes these
types instantiable.

...and disallow inclusion of individual files unconditionally.
