This causes warnings from the notebook and combobox code.

Add annotations to all exported functions in GTK+ headers.

gcc has optimizations for include guards that only work
if they are outermost in the the header.

Sorry for this late ABI break, but the newly added style classes
definitely need padding, so I can just as well add missing padding
globally.

...and disallow inclusion of individual files unconditionally.
