"Subscription Request" dialog is unnecessarily obscure

The "Subscription Request" dialog contains the text "Contact", "Identifier", "Alias", "Groups", and "Select the groups you want this contact to appear in. Note that you can select more than one group or no groups."
Nowhere does it actually say what a subscription request is, or what effect it will have. And not only does it have "Yes" and "No" buttons, it has them without even asking you a question.
The dialog should be rearranged so that:
- it's obvious what the effects of accepting or declining the request will be
- it doesn't need to use the words "Note that"
- it doesn't contain buttons labelled "Yes" or "No".
It's possible that if the choice is described more directly, the dialog won't even need to use the term "subscription" at all.
The first step to fixing this is to redesign the dialog.
